The Maxicare Application procedure is easy.

Step 1: Fill up the Maxicare application form. . You may fill up the Maxicare application from your computer when you open the file. After filling it up, kindly print the form and sign it.

Note:

  1. TIN is always required except for children, students and other similar cases
  2. Height, Weight and Blood Pressure are mandatory information. Normal Blood Pressure is 120/80
  3. On the declaration of existing illnesses, if you are unsure of the exact details and cannot provide specific details and documents if necessary, just mark “No”. Any declaration (YES) might require you to submit additional medical requirements to verify your history.

Step 3: After 1 week, you will hear from the results of the evaluation through telephone/email/SMS

Step 4: On that same week, you will receive a hardcopy of the Maxicare Billing Statement at the billing address you indicated in the Maxicare application form billing address field.

Step 5: Pay your Maxicare Billing Statement. To view our payment centers go to http://tinyurl.com/maxicarepaymentcenters

Step 6: After 1 week, you will receive your Maxicare ID in the billing address you indicated in the Maxicare application form billing address field. Your Maxicare ID will now be ready for use.

6 month probationary period dilemma


This came from Megan….

I'm about to resign in my current employer and about to hop on my next company to work for come next month. My problem is, they don't provide HMO plan until you get regularized, which I think after 6 months.
That is the standard practice in most companies. They offer you the HMO benefit as an incentive or added bonus when you get regularized. For some companies, they do not even offer a salary increase and only offer the newly regularized employee some incentives like the HMO, rice allowances etc. You cannot do anything about this. You can perhaps negotiate this with your employer especially if you are on the managerial level and ask them if they can give you the HMO benefit on your first day. The HMO cannot do anything about this since this is a prerogative of the company. The HMO is just a supplier.

I'm now contemplating as to whether temporarily avail an HMO plan for myself and my parents (51 and 57 years old, who can be my dependents) or not. But, I know that you really can never tell that during my probationary period, which would run for 6 months long to be regularized, I or should I say we, would encounter sickness.
That’s true. No one can tell. So it’s better that you are prepared and cautious. Would you know if your company covers dependents? This is not automatic. Having dependents is not automatic once you have an HMO card. The company pays the HMO company the total headcount including the dependents. Again, the HMO is just a supplier and executes what your company has designed for its healthcare program. If they allow dependents, do they allow single employees to have dependents? Double check. They just might be allowing married employees only. If yes, how many dependents are they allowing.

My questions are:

1.) How would I pay for such plan? Is it gonna be over-the-counter thing?
I am not sure about the practice for other HMOs but for Maxicare, you can pay your dues at any branch of BDO (in cash) or at the head office either cash or credit card.

2.) I browsed some *top HMO providers websites, but cannot seem to grasp everything. How much ba talaga ito every month? Or every year ang payment?
I am not sure if other HMOs have a monthly mode of payment. For Maxicare, it’s at least quaterly. The rates are based according to the choice of plan, age bracket and mode of payment. Check out http://maxicareinquiry.blogspot.com for more info

4.) Like how much is the range kung monthly? or kung yearly? Halos pare-pareho lang naman ata sila, give me a sample range pls. (pero sana monthly)

3.) What's the most affordable HMO provider to apply for, yet ok ang coverage?

4.) After 6 months, I’ll then be entitled for an HMO corporate account under my company, would I be able to terminate my HMO individual account then?
For Maxicare yes you can if you choose the semi annual or quarterly mode of payment. Just don’t pay your bill and your coverage is automatically terminated.

Download the MAXICARE Individual and Family Program

Embedded in this page is the MAXICARE Healthcare Program for the Individual, Family accounts. You can download the presentation by clicking on the download button located at the upper left corner of the presentation.

There are basically 4 plan types: Platinum Plus, Platinum, Gold and Silver.

Each plan differs from the room and board accommodation, Maximum Benefit Limit and Annual Check Up Package. Payment Mode can be Annual, Semi Annual or Quarterly.

The MAXICARE Billing Statement

You will receive a hardcopy of the Maxicare billing statement through your declared billing address which is sent to you approximately 15days to 1 month before your due date. The Maxicare bill is due 15 days from the statement date. Failure to pay means cancellation of membership for New Maxicare Members. A penalty fee of P500 will be charged for existing Maxicare members. The advantage of going through a Maxicare agent like me is that you will have someone that will remind you of your Maxicare due dates.

The Maxicare bill can be paid at any branch of Banco De Oro. It’s a good thing that in some malls, they have BDO that are open on weekends. When your Maxicare due date falls on a weekend, the Maxicare due date is moved automatically on the earlier working day which is usually a Friday. Once you visit the BDO branch use their Bills Payment Form and fill up the following details:

Account Name: Your Name eg. Juan dela Cruz

Account Number: This is the reference number located at the upper right portion of your billing statement eg. ref 2-00000-1-00006821

Please take note that your reference number varies per Maxicare billing statement so on your next Maxicare billing statement, kindly copy the reference number from the current billing statement always.

Should you want to pay through credit card, you may visit the Maxicare Head Office at 19F Medical PlazaBuilding, Amorsolo cor Dela Rosa Streets, Makati City (back of Makati Medical Center)

From payment, kindly expect delivery of your Maxicare ID within 1 week and you can already use your MAXICARE card.

MAXICARE APPLICATION FORM, the simplest requirement

When you have finally decided to enroll under the Maxicare Individual and Family Program, the simplest, basic and maybe the only initial requirement is the Maxicare application form. Embedded in this page is the MAXICARE Application Form for the Individual, Family accounts. You can download the Maxicare application form by clicking on the download button located at the upper left corner of the presentation. Once you receive the Maxicare electronic application form, you can fill up the Maxicare application from your computer once you open the file. After filling it up, kindly print the Maxicare application form and sign it. Maxicare Application forms without a signature will not be processed.

Spending on healthcare will remain low

Philippine healthcare spending is at a low level, and private spending exceeds healthcare spending by the state. However, the Philippine government is trying to promote the access of poorer segments of the population to healthcare, although government expenditure is constrained by the large fiscal deficit. As a result, poor families’ membership of PhilHealth (the national health insurance programme) has been facilitated by a sponsored scheme, and PhilHealth now claims universal coverage, although many poor families remain without PhilHealth cards.

The share of out-of-pocket spending in total expenditure, an important indicator of the inequality of the distribution of healthcare charges, fell steadily from 1996, reaching 41% in 2000, but rebounded to 48% in 2002, no doubt reflecting budgetary constraints on government expenditure increases. Spending by central and local governments accounted for 41% of total healthcare expenditure in 2000, up from 35.2% in 1992, but fell back to 30% in 2002, whereas social insurance schemes provided 9% of expenditure in 2002, up from 7.1% in 2000 and 3.8% as recently as 1998. Although government attempts to increase healthcare spending will be limited by attempts to shrink the fiscal deficit, it is likely that healthcare will account for a gradually rising share of expenditure over the forecast period. The Economist Intelligence Unit therefore expects the market for healthcare to offer more potential in the forecast period than in recent years.

Healthcare in the Philippines

Introduction

Officially called the Republic of the Philippines, this island nation is situated in Southeast Asia. Its capital is Manila and it has more than seven thousand islands, making it the world’s twelfth most populous country.

Overview of Healthcare

Although some people may have misconceptions, most of them are unfounded. The main difference between public and private hospitals are the facilities and technologies offered. There are strict guidelines in relation to prescription drugs; while some countries allow their patients to obtain certain drugs without a prescription, the Filipino pharmacists have stringent directives in relation to the sale of such drugs.

Cost of medicines and hospitalisations

Generally speaking, hospitals are not that expensive in the Philippines. Medicines are also affordable; both locals and expatriates will attest to this fact. Additionally, in the Philippines, almost ninety percent of the population can speak and understand English. Communication is never a problem when you are in the Philippines. The people are very hospitable and accommodating, the doctors and medical practitioners are friendly and the place is very inviting.

Overall, the healthcare system in the Philippines is affordable, the doctors are well-trained and the nurses are of a high standard.

Health issues in the Philippines

The Philippines have been struggling with some healthcare issues, one of the most noticeable being illegal drug use. There are a number of Filipinos who are said to be addicted to prohibited drugs. However, the Philippine government has concentrated on this issue and although there are still some recorded incidents of drug abuse, the number of drug dependents has gradually decreased.

Another serious health issue is malnutrition. Unsurprisingly, the poorer citizens are the ones who suffer from this condition. The Philippine government, through the Department of Social Welfare and Development and in conjunction with the Department of Health, has initiated projects to help its poorer citizens who suffer from starvation.

Cases of dengue are also notable. However, most of these cases are properly attended to by the Philippine government.

MAXICARE Family Accounts are cheaper than Maxicare personal accounts

Some families would find Maxicare’s Family account more advisable than getting an individual account because the Maxicare family rates are cheaper by approximately 10-20% than compared to Maxicare’s Individual Accounts. The misconception in the market today is the term dependent. Dependent connotes a sharing of the Maxicare health benefit. The root cause I suppose is that in Maxicare corporate accounts and in other HMOs I suppose, employees are entitled to enroll dependents which gives the impression that a family account would automatically allow the Maxicare principal enrollee to add Maxicare dependents. The truth actually is that the Maxicare fees are paid on a per head basis and the company actually paid Maxicare the total number of heads including those of the dependents.

With Maxicare’s Family program, we need at least 2 family members, one (1) principal (21-60 years old) and one (1) dependent, again the Maxicare fees are charged on a person basis and according to their age bracket.

To clarify you better on Maxicare’s Family Account applications, enrollment must follow an order/hierarchy.

1. If you’re single, you must first enroll your mother > father > eldest sibling > youngest sibling
2. If you’re married, you must first enroll your spouse > eldest child > youngest child

So example, if you are the Maxicare principal and you happen to be single and you want to have a Maxicare dependent, then you should enroll your mother first. If you want to have two (2) Maxicare dependents, then you can now enroll your Mother and then your father. The same goes for Maxicare married applicants, the order of enrolment of hierarchy must be followed. There is no limit on the number of Maxicare direct dependents as long as they follow the hierarchy. Pricing is on a per person basis following the table of Maxicare Family Membership Fees.

Additional Requirements for Maxicare Family memberships:
1. For single applicants
• If enrolling his parents, photocopy of birth certificate of the single applicant
• If enrolling siblings as well, photocopy of the birth certificate of sibling

2. For married applicants
• If enrolling the spouse only, photocopy of marriage contract
• If enrolling the kids as well, photocopy of birth certificate of the child

Remember that there are instances that a Maxicare dependent cannot be enrolled and the usual reasons are:

a. underaged / overaged, We only accept applicants 2-60 years old
b. working overseas
c. deceased
d. already has a Maxicare card or HMO card from other companies

With these, the Maxicare principal applicant can now move to the next dependent according to the order of hierarcy provided that proofs of ineligibility must be provided. The following are the standard documents accepted:

Underaged / Overaged
- photocopy of birth certificate

Deceased
- photocopy of Death Certificate

Working overseas
- employment contract

Already has a Maxicare card or HMO card from other companies
- photocopy of Maxicare card
