We have specific requirements for NGOs, Foundations, Churches, Associations, Cooperatives, Associations For us to quote these accounts here are our requirements:

REQUIREMENTS FOR QUOTATION
1. minimum of 10 employees enrolling
2. Signed Maxicare Prospective Account Form. (see attached)
3. SEC Registration Certificate and/or Cooperative Developing Authority Certificate of Registration (for Cooperatives)
4. Signed Questionnaire for NGOs, Foundations, Churches, Associations & Cooperatives. (see attached)
5.  2 Year Audited Financial Statements
6. Filled out Detailed Masterlist (see attached)

Maxicare Recovery Accounts or those with previous history with Maxicare

Recovery accounts of accounts that have with previous history with Maxicare will be quoted differently. For us to quote these accounts, we have specific requirements. Here are our requirements:

The following enrollment set up can be applied when choosing a plan type for all the employees. Note that the enrollment
