The Maxicare Application procedure is easy.

Step 1: Fill up the Maxicare application form. . You may fill up the Maxicare application from your computer when you open the file. After filling it up, kindly print the form and sign it.

HOW TO SUBMIT THE FORM:

Option 1: (Via Email) You may scan the document and email it back to me.

Option 2: (Via Fax) You may also fax it to us at (02) 751-5164 route code 1017

Note:

1. TIN is always required except for children, students and other similar cases
2. Height, Weight and Blood Pressure are mandatory information. Normal Blood Pressure is 120/80
3. On the declaration of existing illnesses, if you are unsure of the exact details and cannot provide specific details and documents if necessary, just mark “No”. Any declaration (YES) might require you to submit additional medical requirements to verify your history.

Step 3: After 1 week, you will hear from the results of the evaluation through telephone/email/SMS

Step 4: On that same week, you will receive a hardcopy of the Maxicare Billing Statement at the billing address you indicated in the Maxicare application form billing address field.

Step 5: Pay your Maxicare Billing Statement. To view our payment centers go to http://tinyurl.com/maxicarepaymentcenters

Step 6: After 1 week, you will receive your Maxicare ID in the billing address you indicated in the Maxicare application form billing address field. Your Maxicare ID will now be ready for use.
